On the ground wire on your amp, where do you connect it to? Do you connect it to the frame of your truck?

Yes,make sure to scrape or sand down the paint first so its pure metal and a clean surface this will give you a better ground connection. Then you can just drill a hole and put a bolt in it with the ground wire in between the bolt and frame.Make sure it is a good three feet away from the amp.

So I can connect it inside the cab of my truck.

Actually, the ground wire needs to be as short as possible, as close as you can possibly get it while leaving a little slack.

Do you think I can run the ground wire out my door so I can hook it up to the frame?

any metal part of the body should do fine for a ground connection. i'd not run a cable out the door.
