Recently my older Nissan Maxima was broken into and the idiots took just the face plate off the Stereo. I'm not sure what model the stereo was as it was updated from the manufacturer's model by a previous owner. So first, how do I determine the model and second, how and where can I obtain a faceplate?

Those idiots took your faceplate for a reason. Somebody needed your make/model.
The only way you can find out is to pull out the radio and look under or behind it. There is usually a sticker stating make/model.
You probably won't be able to buy a faceplate. They as in manufacturer, does not like to sell parts, just the whole radio.
Manufacturers should match the radio with the faceplate so that they are not interchangeable with another unit. That way, it'll stop people from stealing.
You might be better off to buy a new radio.
