HTS770 Set up questions

I thought I had my new 770 all set up but I have problems with the audio from my Cablevision set top box. Specifically, my premium channels are about 1/3 the volume as others. What a pain to have to adjust volume so drastically every time I change the channel.

My setup is currently thus:

DVD to RCVR coax-audio, Svideo-video

Cable STB to RCVR analog RCA-audio, Svideo-video

Monitor out Svideo to TV

Unfortunately I have no optical outs on my source components so, is there a way to connect both DVD and Cable box digitally? There seems to be only one Coax digital input on the receiver?

Could my analog connection have something to do with my volume difference? Cablevision says everything is broadcast digitally so I can't even determine if the problem is in my hookup or with the cable company.

I assume also that the set-up defines, to some extent, what setting I use for viewing TV (HBO, Football, etc). i.e. the DSP settings like TV-LOGIC vs surround settings like PLIIx or NEO

I am perplexed. Can anyone offer insight?
