On my new Sanyo 8100, I wanted to make sure that I got a hands free that I could wear comfortably for hours. With this one, I can absolutely say that this is the most comfortable hands free device I've ever used. I put it on after I bought it at Circuit City around 7 PM, and wore it all evening long until 3 AM (8 hours straight). I never took it off. I never switched ears. It stayed in my ear the whole time, and there was no discomfort whatsoever. This is a thousand times better than my "good" Nokia ear piece that would occasionally slip out, and would need to be switched from ear to ear because of discomfort. Not only am I recommending this to any person who needs to be on the phone alot - but I'm going to get one for a friend also... it's that good!
I'm not certain how much of a difference the three different inner ear pieces affect performance. I quickly switched out of the short stubby one into the middle length piece and haven't bothered to try the long one.
I believe that I could probably wear this ear piece for 24 hours - maybe even sleep in it if I had to.
I have gotten good reports on audio quality with my Sprint phone. I have not yet tried it while driving in the car, so I'm not sure how well it operates in a noisy environment. Perhaps I'll be able to post an addendum after a month's use.

After one week... I changed my review from 5 to 3 stars ... the ear piece won't stay in my ear, and my girl friend is complaining that hers won't either. The gel part of the ear piece picks up ear grease rather quickly, and slides out almost as fast as you put it in... especially if your jaw is moving. It's still comfortable... but it won't stay in. I also have noticed that the microphone seems to be 2-3 times more sensitive to background noise compared with the 8100's internal microphone.
As of today, I am retiring my Belkin Handsfree, and am testing out a new Sony handsfree piece which seems to be more secure as far as the ear piece staying in or near the ear opening.
